Preparation for hospital discharge of elderly and caregivers: experience report

Abstract

Introduction: The aging of populations and individuals requires special attention from nurses regarding health promotion and disease prevention. Objective: Prepare hospitalized elderly and caregivers for hospital discharge, through the development of health literacy skills, construction of educational materials and performance of health education activities. Methodology: This is an experience report of an extension project that was carried out from August to December 2022, by professors and students of the Nursing Course of the Federal University of Alagoas, in the Medical Clinic sector of a large Philanthropic Hospital, in the municipality of Arapiraca, Alagoas. Results: A total of 26 elderly people and about 30 caregivers were attended. The construction of individual care plans enabled a better understanding of the elderly and caregivers about the necessary care at home. In addition, it was possible to clarify doubts and transmit confidence to perform the guided care.  Conclusion: The scarcity of information about home care is evident, and health professionals are essential to promote care strategies, with emphasis on the role of the elderly and caregiver, in addition to the formulation of health education activities.
